What have we learnt from nearly 2 million remote HIV tests?

Reporting 10,000 reactive HIV results from self-sampling services – how cut-off index helps clinicians manage false reactive results 

Online testing now accounts for more HIV tests done annually in the UK than clinic-based testing, and it will play a crucial role in achieving zero transmissions of HIV by 2030. The 5th generation HIV assay used by Preventx on fingerstick blood specimens is highly sensitive and specific, yet inevitably there are a small number of reactive specimens that do not confirm as true positives on repeat testing.  Having performed nearly 2 million HIV tests and released 10,000 HIV reactive results, Preventx has a substantial dataset that has allowed us to analyse Cut Off Index (COI) results to determine predictive values. This COI is used by our clinical partners in the management and discussion of results with service users, enabling them to manage results in an individualised and patient-centred way, providing greater clarity and less anxiety for service users and clinicians.

Dr Sara Day, Clinical Lead for Sexual Health London, will talk about how they share HIV reactive results to service users, how they now manage those with low COIs and how Preventx data has made a difference to their clinical pathway.

Who we are

Preventx is the largest provider of remote sexual health testing in the UK, working with the NHS and over 70 local authorities. Our SH.UK service enables people to access free sexual health services, including testing, contraception and treatment, and through Sexual Health London we partner with the NHS to run the largest publicly funded remote sexual health testing programme in the world.

test.me offers easy to access at home sexual health testing for anyone who doesn’t qualify for a free STI test.

Our Laboratory

The Preventx in-house laboratory is a UKAS ISO 15189 accredited medical laboratory (no. 7568) and adheres to a strict quality system.

We only process our own tests, from our own sampling kits and with a focus on sexual health. We don’t process tests for anyone else, so no other work is more important or higher priority.

Preventx is the only digital sexual health service with its own in-house laboratory, and we have developed and evolved our processes specifically to test the small samples that are produced through self-sampling. For example, we can carry out up to six tests on a single finger-prick blood sample, plus confirmatory testing and assays such as quantitative RPR.
