1. How will Preventx use my personal information?
To request a free test, you will be asked a number of questions, including some personal questions. This information is collected by us on behalf of
the NHS who are covering the cost of your free test. You may opt out of most questions, and any information you do give will be treated in strict
confidence.
The information may be passed to your local NHS chlamydia screening office to calculate national averages, but will never be used to identify

3. Confidentiality
You local chlamydia screening co-ordinator will ensure that patient confidentiality is maintained within their area of responsibility.
Confidential data contains patient identifiers (clinic or NHS number, date of birth, and postcode). These will not be shared with or
disclosed to anyone other than the provider of the data (Preventx Limited), the local chlamydia screening co-ordinator and the
chlamydia screening data custodian at HPA. No data may be disclosed to any other parties unless in aggregate form and with the agreement
of those responsible for their provision.
Additional confidentiality measures include, but are not limited to, the following:
- The full postal code of residence will only be used to allocate patients into
geographic areas by strategic health authority, PCT, local authority and
ward, after which it will be stripped from the file and kept in a separate,
password-secured database.
- A unique identifier generated at the HPA Centre for Infections will be used to
link screens from the original National Chlamydia Screening Programme
database to
the postcode-specific one. The NCSP reserves the right to release postcode-specific
data under certain circumstances, which are outlined in the data dissemination policy available at:
www.chlamydiascreening.nhs.uk
- While data are held locally, access to records should be restricted to the local chlamydia screening co-ordinator and, where
necessary, a colleague who is in a position to help with the administration under supervision. Restriction of access to such users
must form part of the system security, responsibility for which should be taken by the local chlamydia screening co-ordinator.
- At no time should data be used for any purposes other than those for which they were specifically collected, unless the consent
of the providers of that information has been confirmed.
